Authorisation

I certify that the facts contained in this application are true and correct to the best of my knowledge. I further understand that false statements provided in this application will be grounds for denial of employment or, if already employed, immediate termination of employment.

I understand that the pre-screening process will include reference and employment history verification. I authorise and consent to the pre-employment screening process. I understand that any confidential information I provide in this application, including my social security number and other such personal identification information, will be kept confidential and will not be disclosed without my written authorisation.

I acknowledge that my employment will be contingent upon my cooperating with any additional pre-employment screening procedures required for my position, including a criminal background check, motor vehicle report (MVR), and credit report. I understand that if I fail to cooperate with any pre-employment screening procedures, including failing to sign any necessary written authorisations, my employment with Midwest Comfort Homes will be denied, and any employment offer revoked.

Unless I have expressly indicated that a previous or current employer may not be contacted, I agree that my previous employers may release substantive information, including but not limited to my quality of work, knowledge, productivity, attendance, reliability, working relationships, and any issues of workplace violence. I waive written notice regarding the release of such information. I further agree to hold my previous employers harmless from liability for any statements or information about my previous work performance provided to Midwest Comfort Homes in good faith of its truthfulness.

I understand and agree that employment with Midwest Comfort Homes is at-will and no representative of the company except the President/CEO has the authority to enter into any agreement for employment for any specified period of time or to make any agreement contrary to the at-will status.
